The sheet that I used has 4 columns (Date, Step, Field, Notes) on a sheet called 'data', and one other sheet called fields where I put the slang name of our fields. Below is photos to better explain.
Now you have to create or log into your MIT App Inventor account, https://appinventor.mit.edu/. Then you either have to download the app file I have on this site or go to my project page, https://gallery.appinventor.mit.edu/?galleryid=d98420c1-e244-4317-a858-78d57e5b392e, on the MIT App Inventor and open my project up.
While going through step one you should have downloaded a JSON credential file that gives access to the sheet you created. You will need to load that file into the project as shown below.
