Connect the PS2 pins as described in the sketch, connect a RS232 level shifter to the Serial1 pins on the Teensy / Arduino and the mouse port on the DEC VAX or VT. DEC Mouse pinouts and protocols can be found in the "VCB02 Video Subsystem - Technical Manual" and I will post the relevant info here shortly as well.
I'm super curious to know if this could be modified to be a usb-mouse-to-vt330 adapter.
There are other microcontrollers that have USB host mode which could work for this. I’d suggest looking at using Circle https://github.com/rsta2/circle with a Raspberry Pi Zero to get a quick booting and cheap solution. You would still need an RS232 level shifter for the UART, but $5 for the Pi Zero and $4 for a usb OTG adapter would get you a workable starting point.

Do you have anymore documentation on this one? This has been the best source I have found to try and save some manufacturing equipment. Its turned into a project of interest for me.
