Also LM358 does not have rail to rail output. So your MOSFET might be on.

If +5V is after Q1 , what powers your U1 initially that eventually drives the MOSFET?
Never mind... Your schematic resolution is not enough to see body diode direction. Might also want to crop the pictures and turn off the grids.
