So, I've been working on this off and on for the last year or so, been through three major iterations, and I'm considering a fourth. I've reached all my goals for size and features. The current case doesn't have button pushers, but the switches are there under the surface. The case is 40x40mm, and 7mm thick, excluding the lugs. That could go down as low as 37x37x6mm with thinner plastics, but there's a limit to what my printer can manage. I might have something printed in steel or brass, I think that would be nice.
