This is the Z8 FORTH ROM dump edited to a series of bytes that can be assembled by most assemblers. I used https://www.asm80.com/ which doesn't assemble Z8 source code but will assemble a series of bytes.

Oak Ridge National Laboratory, document TM-10463,
"Optimizing the Zilog Z8 FORTH Microcontroller for Rapid Prototyping"
has a Z8 FORTH presented as an assembler output listing and as a ROM dump. It has OCR text with some errors.

Text and BASIC content as ASCII strings.
A few jump tables identified.
Rest converted to disassembly, which may be code or data.
It generates the known-good bytes, so it is a good starting point.
